Student Patient Chart Guide

This guide is designed for students using ChartFlow's Patient Chart, an educational EHR system for all healthcare disciplines.
 

Chart Basics

 
 
  1. From your home page click the green view activity button to access you activity.
  2. Then, Click the "Patient Chart" button or "Create Patient" button (if it's a new patient activity) to access the patients chart.
  3. After clicking on " Patient Chart", you will be taken to the Visit History page. This is the first page you will see when accessing a patient's chart.
  4. The Patient Header contains essential patient information including Name, Age, Date of Birth, Medical Record Number (MRN), Sex, and Allergies.
  5. The "Return to Activity Dashboard" button at the top right will take you back to the Activity page.
  6. The light blue area of the Patient Header displays Current Visit Information and Visit Navigation options. If there's no active visit, you'll see a "New Visit" button on the left side. For an ongoing visit, an "End Visit" button appears on the right side.
  7. The Left Sidebar contains all of the Chart's Sections, including flowsheets, forms, history, and more. You'll see two types of sections:
    🏥 Patient History Flowsheets:
    Always visible and compile information from all data, whether in a Visit or not. Examples include Patient Details, Visit History, Allergies, and Patient History.
    📊 Visit-Based Flowsheets:
    Only visible depending on the Visit Type selected. The documentation created is tied to the Visit and will only appear on flowsheets or sections within that specific Visit. Examples include Notes, Orders, MAR, Vital Signs, and Assessments.
  8. For pre-built patients, a Scenario Clock may appear at the top of the screen. All dates and timestamps within the patient chart will reference this clock if available.
 

Visit Navigation

 
 
  1. To create a New Visit, click the "New Visit" button in the Patient Header. Note that this option is only available if there's no current active visit or if you've ended the previous visit.
  2. Select the Visit Type, enter the Reason for Visit, confirm or adjust the Visit Start Date, and click Save.
  3. The light blue Patient Header updates to display Current Visit Information and Visit Navigation options. You'll see the following buttons in this area:
    🔄 Manage Visit:
    Modify the Visit Type, Reason for Visit, Visit Start Date/Time, and optionally set the Visit End Date/Time.
    🔚 End Visit:
    End the current visit and enter the Visit's End Date.
  4. Once you've created a Visit, you can click into any section on the left and begin documenting.
  5. Navigate to the Visit History page to review all historical visits.
  6. Use the View Visit button to easily switch between Visits. When you click the View Visit button, the left sidebar will update to show flowsheets and sections for that specific Visit Type. A notification will appear in the top right corner, confirming you've switched to a different visit.
  7. When reviewing a historical Visit, you'll see the Close Visit button:
    🔒 Close Visit:
    Switch back to the Current Visit.
 

Documentation Specifics

 
 
  1. To document in ChartFlow navigate to the desired flowsheet and click the "New Entry" button to begin.
  2. Set the timestamp for your entry. Note that entries must have unique timestamps, with the exception of orders. For instance, you can't record two sets of Vital Signs at the same time. If you attempt to submit a duplicate timestamp, the system will reject the second entry.
  3. You'll encounter the following field types when documenting:
    📝 Free Text:
    Both single line and multi-entry text fields.
    🔽 Dropdowns:
    Single Select (only one entry can be selected) and Multi Select (select all that apply).
    ☑️ Checkboxes:
    Check all that apply.
    ⚡ Actions:
    A green button that appears after initial documentation in fields like Allergies, Immunizations, Orders, MAR, and Appointment scheduling. It allows you to add further details or update the status of your entry.
  4. When navigating through a flowsheet, we recommend using Tab to quickly move to the next field.
  5. After entering all necessary information, click "Save" (always found at the bottom right) to complete your entry.
